Acknowledgement of Parent/Swimmer Code of Conduct

Acknowledgement that families have read, discussed with their swimmer(s) and agree to abide by the AKC Swim Team's Wavier and Parent/Swimmer Codes of Conduct that are posted on the AKC Holy Mackerel Swim Team website. This must be completed before your swimmer attends the first day of practice. They will not be able to get into the water until this is signed (by signing up for the job). Each signature applies to all swimmers in the family. |
